Request for Service - Residence and Respite Unit
Referrals must be made by a social worker and sent to the Medical Records Department of the CRME.

The request must also include the following information:
Purpose of the request;
A summary of the child’s medical file and a recent pediatric report;
A complete psychosocial evaluation;
Gathered information about the child’s profile;
Professional reports from occupational therapists, physiotherapists, audiologists, etc.
For children over 6 years of age who have a developmental disability, a psychological assessment is required.
