Request for Service - Residence and Respite Unit

  1. Referrals must be made by a social worker and sent to the Medical Records Department of the CRME.

  2. The request must also include the following information:
  • ​Purpose of the request;
  • A summary of the child’s medical file and a recent pediatric report;
  • A complete psychosocial evaluation;
  • Gathered information about the child’s profile;
  • Professional reports from occupational therapists, physiotherapists, audiologists, etc.
  • For children over 6 years of age who have a developmental disability, a psychological assessment is required.
